Acute care/general health assessment as needed, including vital sign measurement,

weight measurement, administering first aid for minor illnesses and injuries (e.g.

headaches, stomach aches, sore throats, fever, cuts, sprains, bruises, and minor burns,

etc.).

Treatments and Medications: Permission to administer treatments including, but not

limited to ice/heat administration, eye/eyewear care, eye wash, administer parent-

provided medications including OTC meds for headaches, menstrual cramps, topicals

such as sting relief, burn cream, anti-itch etc., and school provided unassigned Epi Pen or

Naloxone (should a suspected severe allergy or opioid event occur). Signed consent

form and medication will be provided by the parent, unless it’s a school-provided

unassigned emergency medication (Epinephrine or Naloxone).

State Mandated Screenings (vision, hearing, spinal): Mandated screening will be

performed with this consent, to meet Texas screening guidelines. If you choose to opt

out, you must take your child to a health care provider to complete screening and

provide results to the school nurse, or provide a signed/notarized religious

exemption form, or submit proof of an examination within the last year.

Care of chronic conditions /chronic disease management, including, but not limited to

diabetic assessment/treatment, asthma assessment/treatment, severe allergy

assessment/treatment, and developing healthcare plans (in accordance with orders/plans

provided by students' health care providers).

Access emergency support, including, but not limited to notifying Emergency/Traumatic

Injury Response Team, notifying 911 services, communicating with student’s emergency

contacts provided, etc.

Care Coordination: Collaborating with physicians, families, and school staff to ensure

comprehensive care, advocacy, and communicate health information effectively. (i.e.

ensuring all staff are aware of child medical needs and support needed throughout the

school day).

Health education: Educating students and parents on various health topics related to

disease prevention, healthy lifestyles, and managing chronic conditions.

Mental Health Support: Provide care and crisis intervention, and partner with district

counselors for further support if warranted.

Identifying and Addressing Health Barriers to Learning: Recognizing health problems

that may be impeding a student’s educational progress and partnering with education staff

in developing plans to address them. (i.e. participating, advocating in IEP/504 meetings)

Referrals to community resources: Connect students and families with community

healthcare providers and social services as needed.
